Submit a Support Application

Instructions to Submit a In-Treatment Application

  1. Enter the URL  portal.livelikebella.org or click on the “apply via our portal” link from within the “https://livelikebella.org/applying-for-support/” of our website.
  2. On the login page, fill in your username and password and click login.
  3. You will then be emailed a verification code, copy that code into the verification code window, and click “submit code”.
    1. NOTE: If you don’t receive the code within a couple of minutes, you can click on the resend code option to get a new code.
    2. Make sure to check your junk mail if you don’t see the email in your inbox. Emails will be coming from help@livelikebella.org.  You should not send emails to this mailbox as it is an automated mailbox.
  4. Once logged in, click on the “Apply for Support” menu and choose the “In-Treatment support application”.
  5. Complete the form by providing the required information and following the instructions.
  6. Once completed click on the “Save Form” button the submit your request. Please note that if there are errors the form will indicate which fields/section needs your attention.
    1. NOTE: if you require assistance with more than one bill and/or gift card, you must submit an application for each.
  7. After submitting an application, you will receive a confirmation email for the requests.
    1. NOTE: if you provided an email address for a legal guardian, they will also receive an as well with the details. They can then create an account to view the information.
  1.  LLB staff will process requests and if they are approved or denied you will receive emails accordingly.
    1. NOTE: Our process can take up to 30 days.

  5. Complete the form by providing the required information and following the instructions.
  6. Once completed click on the “Save Form” button the submit your request. Please note that if there are errors the form will indicate which fields/section needs your attention.
  7. After submitting an application, you will receive a confirmation email for the requests.
  8. LLB staff will process requests and if they are approved or denied you will receive emails accordingly.
    1. NOTE: Memorial assistance is processed within 48 hours.
